Passport Renewal: Is this what you need? Ask yourself these questions:

Was your passport issued within the last 15 years?
Was your passport valid for 10 years?
Were you 16 or older when your most recent passport was issued to you?
If you answer yes to ALL of these questions, a passport renewal is what you need.

The U.S. State Department requires passport applicants to electronically complete and print Part 2. The completed form will print out with a barcode (Required by Government).
